Community Health in Motion, a collaborative community outreach program, will be offering two free clinics for the Clackamas County community.

FREE Immunization Clinic
Appointments not available, walk-in only
All vaccines required by the State of Oregon for children to avoid exclusion from school will be available. Vaccines are provided free of charge to children on Oregon Health Plan/Medicaid, with no health insurance or with insurance that does not cover the cost of immunizations. Please bring any immunization records you have to the clinic. A parent or guardian must be present and sign an authorization form for children under age 15.

Note: Beginning with the fall 2014 school year, all students K-12 will be required to have two doses of Hepatitis A. Currently, this requirement applies to students K-5, so make sure your middle and high school students are up-to-date.

Uninsured Adults: Flu & Tdap (tetanus, diphtheria and pertussis) vaccines will also be available for all uninsured adults.

Chances are that your child’s heart is healthy – and yet a small percentage of young people have hidden heart defects that put their lives at risk, especially during physical exertion. Play Smart offers a painless, noninvasive screening to check the heart health of young people 12 to 18 years of age.

Providence Heart & Vascular Institute Play Smart screenings include:
Health questionnaire • Electrocardiogram (EKG) • Blood pressure reading • Echocardiogram, if needed
